I give Melisa Noel of Angels & Earthlings consent to perform a Past Life Regression/Life in Between Lives/Higher Self hypnosis session(s). I agree that actual sessions, and the outcomes of these hypnosis sessions are not guaranteed to produce successful results. I understand that the practitioner is not healing me — I am being given an opportunity to heal myself. I agree that the audio footage and information gathered in my session may be used at the practitioner’s discretion for marketing and educational purposes, including but not limited to social media, blogs, books, etc. Personal information will always be edited out for client privacy.
